Self Directed Support

Network for Change can now offer person centred 1-1 support services (modelled on our award winning Outreach Service) to anyone living in the city or county with a direct payment/individual budget.

How it works

If you have been awarded an individual budget or direct payment and are looking for 1-1 support to enhance your recovery and personal development you can contact Network to ask about making a referral to our SDS (Self-Directed Support) Outreach service.

As with our existing Outreach service you will be allocated a key-worker who will support you to develop an individual 1-1 support package (which could also include access to NRC groups and activities) and you will receive support from one of our established community teams.

Support is charged by the hour and invoiced on a monthly basis and can also be provided at evenings and weekends.

Criteria:

To be eligible for our SDS service you should meet the following criteria.

  • I am aged 18 or over (No upper age limit)
  • I am able to cope without 24 hour support
  • I experience serious and persistent mental health problems
  • I may have experienced difficulties in engaging with and relating to support services in the past
  • Support would enable me to take more control over my life and I would like to make positive changes in my life and I am willing to work with Network to achieve this
  • Support would enable me to maintain my independence

Funding:

The SDS service is funded either by personalised budgets or personal contributions.

"The SDS service has been really good for me and really helpful especially the flexibility and having workers support me on a 1-1 basis, which really caters for my needs.  Support was arranged when and how I wanted it, having support at particular times when I need it most, especially at first (leaving hospital).  It's helped me build up my confidence, I feel listened to and the workers support my decisions and choices.  We work together which helps both me and my workers.  I would say to anyone considering this service that you can't go wrong..."
